Part B - Information required

Information details

Please provide details of information you think we hold in the data subject’s name. To qualify as personal data, the data must be biographical in a significant sense, i.e. having the data subject as its focus and affecting the data subject’s privacy. Please supply us with a description of the information required (including departments and reference numbers where known).

Part C - Fee and proof of identity

In accordance with the Data Protection Act 1998 you may be required to pay an administration fee before your information is released. You will be notified if any fee is payable.

We also need to see two documents which prove your identity, such as a passport, driving licence, or a utility bill (but not a mobile phone bill) which should be less than three 3 months old. You may supply the documents by presenting them in person. Please do not send original documents by normal post; you should use a recorded delivery service (and make similar arrangements at your own cost for their return). Alternatively you may send photocopies certified by someone over 18 who is not a relative of the applicant (or of the subject of the application where different). Certification should confirm that the copy is ‘a true copy’, and give the certifying person’s name, address and telephone number. If applying on someone else’s behalf, you must also provide evidence of your authority to make the application and of your own identity.

Failure to provide these documents with your application may mean your request is refused.

With all correspondence please ensure you quote the unique reference number issued for this transaction.

The City of London is committed to the principles defined in the Data Protection Act 1998. As such, information on this document will be used only for the purposes described above. We may, however, store the data in manual or electronic form, but only for as long as we are required to do so by law.
