Please use this form to tell us more about what you have seen. Not all the sections need to be filled in, but ideally we would like to know the answers to WHO, WHAT, WHERE and WHEN.

	WHO: The name of the person who observed the creature.

	WHAT: Do you know what you saw? The following website includes a good identification guide to reptiles: www.herpetofauna.co.uk/identification.asp. If unsure, please describe any details observed in the 'Notes' section below.

	WHERE: Please give the most specific location possible (e.g. Ladies Pond; Top of East Meadow in bramble fringe; Stock Pond Meadow on edge of fence line, near gate).

	Grid Reference: If known, please write an 8 figure grid reference for the location (e.g. TQ286173).

	WHEN: Please record the date the species was seen.

	Time: The time of day the sighting was made.

	Quantity: The number of the species seen.

	Weather Conditions: What was the weather like?

	Notes: Are there any other details which might be of value? (e.g. size of reptile; basking or swimming; description if unsure of species).
