Help for adults

Happy smiling couple The Adult Social Care team has expertise in offering advice and information, support and professional social work assistance to service users and their carers. We provide a confidential service to adults, dealing with a wide variety of issues.

If you have a mental health problem, learning disability, physical disability or problems associated with caring for someone, social workers are able to advise you on the help that is available.

You can receive an assessment of your needs, which may involve discussing with a social worker different areas of your life. After your assessment a social worker will work with you to create a 'care plan' of the help and services you can receive.

Self assessment - If you are interested in any of our adults services you can now refer yourself by filling in the following form and returning it to us.
