To get your freedom pass from the Post Office you will need to
take to the Post Office the following items:
- two passport-sized photographs of yourself (or an existing
freedom pass photocard)
- an existing freedom pass, (with expiry date of 31st March
2006), if you have one.
- one example of proof of name:
- Passport
- Medical Card
- Birth Certificate (unless your name has changed)
- New style driving licence
- Current freedom pass
- one example of proof of address:
- Council/Housing Association rent book
- TV Licence
- Driving Licence
- The following items dated within the last 3 months:
- Utility bill
- Bank/Building society statement
- Inland Revenue letter
- Council Tax bill/letter
- Benefits Agency letter
- Council Tax Payment Book/Benefits Book
If you are applying for an Older Person’s freedom pass you will
also need:
- one example of proof of age
If you are applying for a Disabled Person’s freedom pass you
will also need:
- your authorisation form (from City of London Department of
Community and Children’s Services).
You will then be issued with a plastic wallet containing your
freedom pass and Photocard.
