school place enclosed in the City of London primary and/or
secondary transfer booklets;
You have listed the schools in order of preference;
You have parental responsibility for the child you are applying
for, the child lives in the City of London, and the information
given is true to the best of your knowledge and belief;
You understand that your address details will be checked against
the City of London's Council Tax Department’s records. If your
details are not found, we will investigate further to determine
your address. Your application may not be processed unless we
verify your details.
You must provide a copy of the child benefit letter or other
official government documentation that confirms your child’s
address and date of birth. The document must not be issued more
than a year ago and can either be uploaded online or sent to
us as a hard copy
If you are not able to provide documentation or need to discuss any
aspect of your application, please contact the school admissions
service.
You must inform the City of London of any change in circumstances
(eg change of address) and understand that failure to do so may
make this application invalid or lead to any offer of a school
place being withdrawn;
I understand that any false or deliberately misleading information
given on this form including any supporting information may
make this application invalid, or lead to the offer of a place
being withdrawn;
Data Protection
The City of London will handle the information you have provided
to us in line with the provisions of the Data Protection Act
1998.
Under the Data Protection Act you have the right to make a formal
request in writing for access to personal data held about you or
your child.
Fair Processing Notice
The information you provide on the form will be used by the City
of London for the purposes of administering the secondary school
transfer process.
The City of London has a duty under the Childrens Act 2004 to work
with partners to deliver and improve services to children and young
people in the area, and to comply with the Data Protection Act 1998
in respect of the personal information you provide.
The City may use this information for other legitimate purposes and
may share this information where necessary with other bodies
responsible for administering services to children and young people
or where otherwise required by law.
The City also has a duty to protect the public funds it
administers, and to this end it may use the information you have
provided for the prevention and detection of fraud.
